Delayed hemolytic transfusion reaction by anti-Kpb (Kel4). Report of the first case of anti-Kpb in Mexico

ABSTRACT
We report the clinical and laboratory data of a patient who had a delayed hemolytic transfusion reaction caused by anti-Kpb undetectable in pre-transfusion compatibility testing, and a second acute transfusion reaction by transfusion of a packed red cell unit mistakenly identified as compatible. It was necessary to involve of an American reference laboratory and the Rare Donor File for the management of this patient. This case is presented because of the very low incidence of this antibody. Additionally, this is the first time that an anti-Kpb has been identified in Mexico.
