Congenital bronchobiliary fistula

ABSTRACT
Bronchobiliary congenital fistula is an extremely rare embryological anomaly that includes a malformation of the airways and the digestive tract. The main symptoms are: repetition pneumonia and bile stained sputum. We communicate the case or a 2 month year-old male patient who suffered vomit of some gastric content, cough, and malnutrition (third degree). At physical exploration, generalized hypotrophy, breathing difficulty, inspiratory crackles and hardly breathing were observed. There were no lesions at the esophagogram. Endoscopy that included esophagus and stomach only revealed bile-stained material. The carina showed three holes all bile stained; two were interpreted as bronchi and the third one was considered as an accessory left bronchi. Clinically, the patient had unfavorable evolution and died. Autopsy showed a bilateral pneumonia associated and a fistula from the liver to the carina. The bronchobiliary fistula is a congenital and rare anomaly that produces repetition pneumonia and causes sepsis and death if it is not diagnosed timely.
