Large craniofacial midline defects: treatment with TRAM microvascular transference

ABSTRACT
To remove defects in centrofacial area represents a huge challenge for any surgeon because he/she needs to decide the most effective method for the reconstruction of this area. The most difficult defects to remove are the ones related to carcinoma, in the case of head and neck the most frequent is: carcinoma of squamous cells, basocelular and cystic adenoid, as well as melanoma. For these big defects muscle cutaneous flaps as dorsal and abdominal rectus among others have been used. A of a male case with squamous cell carcinoma of the hard palate with infiltration to the nasal area and left maxillary sinus which required block resection with reconstruction and a cover of the resulting large defect by micro transference of muscle cutaneous free flap of the abdominal rectum was reported. It was successful and achieved 100% of the total integration.
