Suboccipital concentric craneotomy as variant in the posterior fossa surgery

ABSTRACT
Early in the neurosurgery for the posterior fossa approach was described in 1903 by Fedor Krause and it continued by Dandy; the suboccipital approach is based in craniectomy or craneotomy, to emphasize inthe surgical anatomy of the posterior fossa, but with risk for the venous sinus. We discuss the evolution of the suboccipital approach, and we described the concentric variant to improvement the technique.
Clinic material and results: from May 2005 to February 2006, we make suboccipital concentric craneotomy in three consecutive patients, without dural injury, sinus venous preservation, and suboccipital area reconstruction.
Discussion: in the neurosurgery history and the posterior fossa approaches we presented and too a variant, the suboccipital concentric craneotomy that can improvement the surgical technique in the posterior fossa surgery.
Conclusion. We feel that the variant suboccipital craniotomy concentric for posterior fossa surgery, permit optimal exposition and sure and avoid risks in the venous sinus preservation. But is will continue her application in more patients to reaffirm these results.
