Secundary hemophagocytic syndrome from Epstein-Barr virus (EBV): Case report

ABSTRACT
We describe the case of a 7 year old child who presented HPS without any important history. The clinical pattern starts with fever up to 40°C, abdominal pain, acholia, choluria, adynamia, vomit and drowsiness. The patient is treated with Ceftibuten and Nimesulid. During her stay in the health center were performed laboratory tests: TB 6 mg/dl, DB 5.5 mg/dl, LDH 2,610 UI/l, WBC 1000 mm
3, neutropenya, platelets 40,000 mm
3. She was transferred to the Centenario Hospital Miguel Hidalgo and diagnosed with hepatitis and secondary encephalopathy. At the hospital she presented fever, jaundice, hepatomegaly, gastrointestinal tract bleeding, ecchymosis and petechias. She was admitted to the UCIP where treatment begins with Ceftazidima + Amikacina. The abdominal ultrasound presented suggestive data of Colecystitis and cholangytis. Later she presented right hemotorax and pachypleuritis as a complication. The diagnosis was supported by the presence of hyperbilirrubinemia, hypertrigliceridemia, transaminasemia, pancytopenia, the bone marrow aspirate test revealed decreased cellularity, vacuolated monocytes and cells with plentiful cytoplasm, eccentric nucleus and cells with histiocytc look with platelets phagocytosis and isolated leucocytes. Treatment begins with steroids, etoposide and ciclofosramida and she presented favorable response.
