ABSTRACT
Hepatic rupture is one of the most serious complications of pregnancy and can cause death of the mother and the product; together with preeclampsia-eclampsia are the leading cause of death in many countries. Objective: To determine the maternal morbidity and mortality associated with liver rupture or hematoma by preeclampsia-eclampsia or HELLP syndrome during the gravid or puerperal state. Material and methods: We reviewed the medical records of all patients in gravid or puerperal who had hematoma or hepatic rupture associated with preeclampsia-eclampsia or HELLP syndrome in the Hospital of Gynecology and Obstetrics IMIEM the period January 2003 to June 2009. Results: Of the 2,555 cases of severe preeclampsia or HELLP syndrome, 5 (0.19%, one per 511) patients had hepatic rupture, or hematoma of the liver, one per 11,000 live births. The average age was 30.3 years. The mortality rate was 40%. Conclusion: Early recognition, correct diagnosis and appropriate multidisciplinary management of hepatic rupture associated with preeclampsia and/or HELLP syndrome should reduce mortality.
