Utility of scintigraphy with Indium-111-labeled leukocyte activity and complementary Technetium-99m-sulfur colloid

ABSTRACT
Introduction: Infections represent 1-2% of all complications following arthroplasty. Detection of osteomyelitis is still challenging for modern imaging. Gold standard for detection include imaging with
111In-labeled leukocytes and
99mTc-SC, with a reported accuracy varying from 88% to 98%.
Objective: To demonstrate the utility of
111In-labeled leukocytes and
99mTc-SC imaging for detection and follow-up of osteomyelitis.
Material and methods: Data from 111 patients with clinical suspicion of osteomyelitis which also had imaging studies with
111In-labeled leukocytes and
99mTc-SC were obtained. Osteomyelitis was considered positive when no spatial correlation of
111In-labeled leukocytes and
99mTc-SC-labeled bone marrow was observed, whereas spatial correlation of both images was considered negative for bone infection.
Results: Inclusion criteria was met only in 11 patients. Statistical analysis showed 100% sensitivity, 85.71% specificity, 80% PPV, 100% NPV and 90.90% accuracy for osteomyelitis detection.
Conclusions: Based in our statistical results, no spatial correlation of
111In-labeled leukocytes and
99mTc-SC-labeled bone marrow is a reliable method for detection and follow-up of osteomyelitis.
111In-labeled leukocytes and
99mTc-SC imaging demonstrates ideal sensitivity and NPV for osteomyelitis detection, strongly supporting its use in clinically suspected cases. Negative results virtually exclude bone infection with no need of additional studies.
