Hypertensive disorders in pregnancy

ABSTRACT
During 2004, in Mexico the maternal mortality rate was of 62.2 deaths/100,000 live newborns. Being the hypertensive disorders during the pregnancy one of the main etiologies. Classified in preeclampsia-eclampsia, hypertension alone or with preeclampsia superimposed and gestational hypertension. The preeclampsia has a prevalence of 7% in pregnant women. One of the most accepted explanations is that the incomplete invasion of the trophoblast, provokes in the maternal endothelium a vascular response. With the early diagnosis based in a correct clinical and laboratory analyses, considering the differential diagnosis (acute fatty liver of the pregnancy, systemic lupus erythematosus, uremic hemolytic syndrome, thrombotic thrombocytopenia purpura, heparin induced thrombocytopenia). The primary prevention is based in changes in lifestyle in case of preexisting high blood pressure, as well as pharmacological management with the specific drugs for a particular clinical circumstances to decrease the complication (thrombotic microangiopathy and intravascular hemolysis, eclampsia) and the resultant cause of maternal and perinatal morbidity and mortality.
