ABSTRACT
Objectives: Identify the effectiveness of treatment in the postpartum HELLP syndrome with dexamethasone and methylprednisolone in patients admitted to the Intensive Care Unit of the Hospital of Gynecology and Obstetrics of Instituto Materno Infantil del Estado de México (IMIEM) 2004 - 2008.
Methodology: A study of series retrospective and cross-case comparison of records of patients with HELLP syndrome (Classification Tennessee). The population was classified into two groups: one received treatment with 10 mg of dexamethasone every 12 hours over three days and the other received 1 g of methylprednisolone every 24 hours for three days. For the analysis of results using descriptive statistics and Student t.
Results: We studied 79 patients, of whom 54 (68%) were the group that received dexamethasone and 25 (32%) methylprednisolone. We performed a comparative statistical analysis found a statistically significant difference in mean arterial pressure (dexamethasone 101.56 mm Hg
versus methylprednisolone 95 mm Hg, p 0.03). There were no statistically significant differences in the numbers of platelet count, transaminases, lactic dehydrogenase, total bilirubin, as well as days of stay in the intensive therapy unit and number of antihypertensive drugs at discharge of patients from service.
Conclusion: There was no statistical difference significant in most of the studied parameters except for mean arterial pressure, it is suggested to perform further randomized double-blind studies to justify the use of steroids in the puerperium in patients with HELLP syndrome.
