Cryptococcosis in AIDS age

ABSTRACT

Cryptococcus neoformans produces meningitis in patients who are immunocompromised with the acquired immunodeficiency syndrome (AIDS). Between 6% and 13% of patients with AIDS are infected with Cryptococcus, and it is the initial opportunistic infection in about half of all AIDS cases. C. neoformans is an encapsulated yeast like fungus that reproduces by budding and grows at 37°C, it hydrolyze urea during growth on Christensen agar. Another useful laboratory procedure is based on the fungal property to produce dark brown pigment (melanin) in agar media made with glucose, creatinine and supplemented with Niger seeds. There are DNA probes for hybridization with RNA of organism, it is a very sensitive and specific method and may in future replace biochemical identification procedures. Latex agglutination test detects the polysaccharide capsular antigen in the cerebrospinal fluid, and when tissue specimens are obtained cryptococcosis may be diagnosed by histopathology and/or blood culture.
