ABSTRACT
This is review of the most relevant evidence available on HIV/AIDS-associated tuberculosis, considering the severity of the epidemiologic situation and the devastating consequences that coinfection with both pathogens produces on patients. MEDLINE, PUBMED, as well as a number of available publications in our Medical Sciences Information Provincial Center were reviewed. This work consists of an introductory explanation of the epidemiologic behavior and the current situation of the AIDS pandemic and the consequent reemergence of tuberculosis as a results of the progressive immunological deterioration of these patients. Later on we discuss what we have named: the target of the tuberculosis/AIDS binomial. This item consists of three primary pathways. The first one relative to the early progression of HIV infection during and after diagnosis of ongoing tuberculosis; the effects of tuberculosis on AIDS, and the molecular and pathogenic grounds of these observations which lead to the question: Are these evidences enough? We then reviewed the challenges for the diagnosis of tuberculosis in these subjects with special interest on tuberculosis without microbiological proof, undoubtedly an unquestioned reality and in a way attributable to the AIDS pandemic. Finally, we dedicate some space to the implications of coinfection-coillness on the therapy of both tuberculosis and AIDS (highly-active antiretroviral therapy), which obeys to the multidrug resistance of tuberculosis, the so-called “third epidemic disease”, a reality that constitutes a nosocomial threat even in populations unaffected by HIV/AIDS.
