Prevalence of intra-abdominal hypertension and abdominal compartment syndrome in critically ill patients

ABSTRACT
Objective: To determine the prevalence of intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) in critically ill patients.
Patients and methods: We prospectively studied consecutive patients admitted to Intensive Care Unit (ICU) from June to September, 2008. Intra-abdominal pressure (IAP) was measured transvesical at admission and each 6 hours during the first 24 hours of stay in ICU. Demographic and clinical variables were recorded and prevalence of IAH and ACS was estimated. We recorded length of stay and mortality in ICU too.
Results: We analyzed 52 patients, 50% women, mean age 51.35 years. Most frequent risk factors for IAH were mechanical ventilation, renal failure, massive fluid resuscitation, acidosis and hypotension. IAH was present in 59.6% patients while 3.8% developed ACS. Eighteen patients has IAH grade I, 11 grade II and 2 grade III. Mean length of stay in ICU was 6.5 days and mortality was 21.1%.
Conclusions: Prevalence of IAH in critically ill patients is high. Grade I of IAH is the most frequent. Probability of death is higher with higher grade of IAH. IAP should be measured routinely in critically ill patients.
