Metabolic state before and after bariatric surgery at Centro Médico Nacional 20 de Noviembre

ABSTRACT
Introduction: Surgical treatment of morbid obesity is an efficacious means of solving or at least significantly improving the comorbidities associated with this condition.
Objective: To report the metabolic changes occurring in patients subjected to bariatric surgery at Centro Médico Nacional 20 de Noviembre.
Patients and methods: Descriptive, retrospective and comparative study, based on chart review. A database was created which included age, gender, anthropometric measures, blood glucose, insulin, HOMA, total cholesterol, LDL-C, HDL-C and triglycerides at baseline as well as at 1, 3, 6, 9 and 12 months after surgery.
Results: Fifty eight patients were included, 74% underwent restrictive surgery whereas in 26% a mixed restrictive-malabsorptive procedure was performed. At one year follow up, the mixed procedure achieved a greater degree of weight loss than the restrictive technique (47 kg vs. 39.8 kg). Glucose and triglycerides levels, as well as HOMA index changed significantly overtime from baseline to the different postoperative evaluations (p = 0.000, p = 0.002, p = 0.002, respectively). Pharmacological treatment could be withdrawn in 83% of diabetic, 65% of hypertensive and 84% of dislipidemic patients. Global mortality was 6.7% and always due to surgical complications.
Conclusions: Bariatric surgery significantly improves the control of the different co-morbidities of morbid obesity.
