Primary ovarian pregnancy and hyperstimulation syndrome resolved by laparoscopy

ABSTRACT
Introduction: Ovarian pregnancy is an uncommon variety of ectopic pregnancy in which the gestational sac is implanted in the ovary. The development of an ectopic ovarian pregnancy followed by an ovarian hyperstimulation syndrome is a rare condition with only two cases reported in the literature.
Case report: A 28 year old female with secondary infertility, had ovulation induction based on clomiphene citrate and human chorionic gonadotropin hormone. Five weeks after the last menstrual period she had abdominal pain, colic type, intermittent, in both right and left illiac regions. Pelvic ultrasound demonstrated left ovary with a gestational sac of 5.6 weeks, ovaries increased in size and free fluid. Laparoscopic surgery was performed confirming the diagnosis; surgery preserved the entire ovary.
Conclusions: We should have a close assessment of patients who are undergoing ovarian stimulation and consider the possibility of an ectopic pregnancy because symptoms in these patients are generally more severe.
