ABSTRACT
Reduction of the bony structure of the face is the main reason for flabbiness and fall of the facial soft tissues. Because we do not have resources to restore the lost bony volume, a good possibility is to reestablish a good relationship between the soft tissues and the present bony volume. Based on this principle a procedure of mini-invasive subperiosteal facial restoration has been developed. In this paper the frontal mini-invasive ritidoplasty procedure is described as a result of experiences and observations throughout ten years. Since 1998 this procedure has been performed with several combinations and changes depending on the needs of each particular case, in 521 patients: 419 female and 102 male, from 45 to 73 years old. The aims and means used to carry out this procedure: frontal, palpebral or combining both approaches are described. Based on the surgical anatomy, guidance is given to reach the goals of the procedure and to avoid complications. Auxiliary procedures, which can be associated to this procedure, are described as well as the details of postoperative care. The observed complications are mentioned and a summary of the results obtained, which in general have been very satisfactory for the patients and the surgeon. The observations from our experience with this method and the reasons why this is the first procedure chosen, are exposed.
