Three years experience in diabetes ketoacidosis at the Hospital General «Dr. Nicolás San Juan», Toluca, México. Is there a prognostic factor?

ABSTRACT
Background: Diabetes constitutes 35% of all revenue to Internal Medicine, of these, 10 to 20% have some kind of acute complication. In our hospital we have no epidemiological studies on the matter.
Objective: To determine the morbi-mortality in patients with diabetic ketoacidosis admitted to the Division of Internal Medicine at the General Hospital Dr. Nicolas San Juan during the period from January of 2004 to December of 2007.
Material and methods: During March and May of 2008, were located in database all records with diagnosis of diabetes, were selected if they met criteria for diabetic ketoacidosis, and these were analyzed, age, sex, type of diabetes, time of diagnosis, treatment, gases, electrolytes, blood chemistry, stay, evolution and complications. The data obtained were analyzed by descriptive statistics, percentages and averages of Student t test in the program EXCEL 2007.
Results: We analyzed 1,497 cases, of which 52 met criteria of diabetic ketoacidosis, the average age was 40 years, the male predominance in 57.6%, and type 2 diabetics at 63.2%, in the severity, the majority presented a moderate stage in 44.2%, the main trigger were the infections in 46.3%, the pH average was 7.10, bicarbonate of 6.21 mmol/dL, glucose of 403 mg/dL, creatinine of 1.7 mg/dL, sodium of 134 and potassium 3.9 mEq/L respectively, mortality amounted to 15.4%.
