Lower-limbs secondary lymphoedema: Presentation of a problem case

ABSTRACT
Lymphoedema is characterized by swelling, especially of subcutaneous tissues, secondary to the stasis of lymphatic fluid in the interstitial space. It is an angiopathy of difficult diagnosis and treatment because its structural and functional peculiarities. In here we present the case of a 40 year old patient, a chronic alcoholic man with the following antecedents: a lumbar- cuts; a traumatic fracture of the right femoral neck; bilateral- thrombophlebitis of the ilio-femoro-popliteal region of no identifiable cause, and secondary lymphoedema. He had been previously treated with an esplenectomy for the wound mentioned; he had been done an instrumental correction of the fracture; he had an intracaval filter placement of unknown indication; The treatment we gave him was a “Lympho-venous anastomosis” and afterwards a “Dermo-lipo-limphoedema-excisions”, done with dermal-small print - flaps or large flaps collocation for covering the underlying tissues. As a result we observed volume diminishing, and aesthetic and functional improvement. The lymphoedema, either of primary or secondary origin, is of difficult management, wich is confirmed by the great variety of techniques developed to reconstruct the lymphatic pathway, or to make plastic corrections of the affected and dysfunctional limbs. In here, we emphasize the need of a mixed treatment and the use of small print dermal grafts from a healthy zone.REFERENCES
