ABSTRACT

Rhinoplasty is one of the most difficult technique procedures in otorhinolaryngology, this is because every surgical step or modified nose region affects all the structures and the final result. One of these surgical steps is the one related to the nasal tip (usually, patients ask for a better aspect of the nose profile) which takes two phases, one of them determines the height and characteristics of the nasal dorsum, and the other establishes the rotation and projection of the nasal tip. The latter is the objective of this work. Otorhinolaryngologists and plastic surgeons have described several techniques in order to improve this important topic. There is a special area in the particular case of nasal tip, we call it columnello-septal complex, formed by the septum caudal border, inferior–lateral medial cruras, and the soft tissue localized at the columnella region. The correct handling of this complex through fixation techniques allows esthetic and functional results.
