ABSTRACT
Eight-two patients from 0 to 13 years old, with fracture of the femoral diaphysis were treated. They were managed under two types of treatment: 1) spica cast initially placed or 2) Spica cast with previous skeletal traction, depending on the initial displacement at their arrival to our hospital. The follow-up consisted of out patient visits with a minimum average of 32 weeks and frequent x-ray control during the first weeks. The results in patients under 6 years old were similar in both groups: no one presented a shortening of more than 2 cm; nor an angular deformity of more than 15°. In patients older than 6 years old we had: in the initial spica cast group, 8 patients (16%) with an angular deformity of more than 15°; in the spica cast with previous traction group, 2 patients with shortening of more than 2 cm and 18 patients with an angular deformity were obtained in patients over 6 years old, where a different type of treatment is suggested.
