ABSTRACT
Introduction: The variety of dentofacial deformities involving the maxilla has contributed in the design of different techniques for its correction, according to the characteristics of each patient, and having each one of the techniques a specific indication. The conventional Le Fort I Osteotomy used in cases of impactation is not the ideal technique for obtaining favorable results regarding lip competition besides producing breathing problems by diminishing airways capacity.
Methods: A case of 16 years-old male patient is presented, who had a 10 mm maxillary vertical excess. The total alveolar Le Fort I technique or horseshoe technique in two segments was carried out, as a surgical treatment.
Results: By using the total alveolar technique, it was observed it was the ideal treatment, to rehabilitate function and to improve aesthetics, for patients with a maxillary excess superior than 5 mm.
