Upper gastrointestinal bleeding. Medica Sur Clinic & Foundation Experience

ABSTRACT
The upper gastrointestinal bleeding (UGB) is a frequent disease that determines morbidity and mortality. The aim of this study is to analyze the main clinical variables in patients who attended to the emergency service in this hospital, during a 12 month period.
Material and methods: Is a retrospective observational study. Clinical files were evaluated; we gathered demographic data, clinical endoscopic diagnosis, evolution, and hospitalization data. Patients with variceal bleeding were excluded.
Results: 102 patients in ages between 15 and 94, with an average of 51.6 years old. They received medical treatment with the usual measures of support and stabilization. And have urgent or immediate endoscopy. The causes of UGB were: acid peptic disease including erosion and ulcers of the three segments 90%, Mallory Weiss 5.8%, other causes 3.9%. Eighty patients (78.4%) were hospitalized, of these 10% entered in shock state. The hospitalized days in average were of 4.3 days, the 43.45% required blood transfusion and were necessary surgery in the 1.9%. Global mortality was in the 3.9%.
Conclusions: 1. The acid peptic disease is the most frequent cause of UGB, including erosions and ulcers of three segments. Followed by Mallory Weiss and others; 2.- The mortality is low when compared with the reported in the reviewed literature. 3.- The urgency endoscopy may avoid unnecessary hospitalization in low risk patients, it probably shortens the hospitalization period and reduce morbid-mortality.
