Angiofibroma nasofaríngeo juvenil con invasión orbitaria bilateral. Hallazgos ecográficos. Reporte de un caso

ABSTRACT
A 12-year-old male with a six months history of recurrent epistaxis, epiphora, left proptosis and dyspnea is presented. Ophthalmic examination revealed: visual acuity 20/20, normal ocular movements in both eyes and exophthalmometry: 17/21. Anterior and posterior segments presented no alterations during the exploration. IOP: OD=10 mmHg, OS=11mmHg. The patient was referred to the Echography Service for evaluation. A bilateral B screening exploration was performed appreciating a heterogeneous mass coming from the middle line and extending into both orbits. In the left orbit the lesion had a superior, nasal and inferior location and in the right orbit the lesion was only nasal and inferior. The anterior border of the lesion was irregular, well defined, with heterogeneous density and evidence of bone erosion. Standardized A scan screening revealed a very low reflectivity lesion with irregular internal structure and no vascularity. The patient underwent an incision biopsy and the final histopathological diagnosis was nasopharyngeal angiofibroma. Clinical and echographical features are discussed.
