ABSTRACT
Central retinal artery occlusion produces an ischemic infarct of the inner two thirds of the retinal layers. Most common etiological factors are vascular emboli from the head or left heart conforming an emergency, which is not very frequently encountered in the ophthalmologic clinical practice.
This report documents the clinical and fluorescein angiographic findings in a subject with carotid artery insufficiency.
This patient was initially seen with an acute central retinal artery occlusion, optic disc neovascularization and amaurosis fugax. The ophthalmoscopic findings showed whitening of the posterior pole, arterial attenuation and pale optic disc.
Serial fluorescein angiograms showed a nearly total shutdown of choroidal and retinal perfusion and eventual development of a huge optic disc neovascular frond. This suggests a previously existing ischemic oculopathy with a superimposed acute central retina artery occlusion.
