ABSTRACT
Objective: To determine if cataract extraction via pars plana or clear cornea plus vitrectromy causes different corneal damage degrees, and describe the complications.
Design: Prospective, comparative, randomized study, including patients with vitreous hemorrhage secondary to diabetic retinopathy plus cataract. Patients were randomly assigned to be treated with lensectomy plus vitrectomy (group A), or clear cornea phacoemulsification plus vitrectomy (group B).
Results: Eleven patients were enrolled. During surgery midriasis was reduced in group A from 7.25 mm to 6.5 mm, and in group B from 7.00 mm to 5.25 mm. During follow-up there were no significant differences concerning inflammation and intraocular pressure between the two groups. Endothelial cell count in group A had a loss of 8.3% versus 22.1% of group B (p‹0.005). Corneal mean thickness was 594 µm for group A and 592 µm for group B. One patient of group A developed postoperative rubeosis and intraocular lens dislocation.
Conclusions: There is no difference between the IOP and inflammation at the preoperative and postoperative time. The phacoemulsification induced a major degree of myosis and endothelial damage. We observed that lensectomy causes an IOL descentration.
