ABSTRACT
Introduction: Eyelid retraction is an unfortunate consequence of cosmetic surgery, trauma or disease. The management of this pathology has been described by many authors with different techniques, especially with the use of spacers. In this study we present a new technique based on the use of a muscle-fat flap rotation of the upper eyelid to the lower eyelid.Purpose: To verify if the upper eyelid muscle-fat flap rotation is useful to repair the lower eyelid retraction.
Material and methods: We included 11 patients, 16 eyes with lower eyelid retraction. The patients attended the Hospital para Evitar la Ceguera en Mexico between February 2004 and January 2006. All patients were submitted to surgery with the rotation of a muscle-fat flap of the upper eyelid to the lower eyelid. Before and after surgery we measured scleral show, space between eyelids, corneal exposure and lagophthalmos. The patient follow-up was made during the first, third and sixth month.
Results: Of the 11 patients, 6 were males and 5 females; the mid age was 52.45 years. The pre-operatory diagnosis was thyroid orbitophaty in 7 patients (11 eyes), two patients with Hansen disease (3 eyes), one patient with lower eyelid retraction due to plastic surgery and finally a patient with facial palsy. The scleral show and eyelids aperture was diminished with a statistic significance of p =0.05. Conclusions: The rotation of a fat-muscle flap of the superior eyelid to the lower eyelid is effective to diminish scleral show.
