ABSTRACT
The thoracic vascular injuries has a high mortality in penetrating trauma; in many cases the surgical approach is difficult; one option is the bilateral thoracotomy with esternotomy known as transverse incision Clamshell that allows us to display a wide mediastinal structures and Pleural. This is a case report with injury fell from the vein azygos and upper lobe lung and a half, which was able to adequately control, after which was left bleeding for presenting packaged important sites further dissection of hypothermia and acidosis, unpacking at 72 pm without complications. With proper evolution. The interest of communicating this case is to give an option of surgical approach, in thoracic vascular lesions.
