Aplicaciones del aloinjerto de dermis humana acelular en cirugía plástica oftálmica y reconstructiva

ABSTRACT
Purpose: To describe the uses and experience of a commercially available acellular dermal allograft (Alloderm®) in oculoplastic surgery in the Servicio de Párpados, Órbita y Vía Lagrimal del Instituto de Oftalmología Conde de Valenciana.Methods: It was a retrospective, noncomparative case series between January 2001 and June 2005. We identified 30 cases (23 patients). Outcome measures included applications of Alloderm, degree of clinical improvement, complications and follow up.
Results: The mean age was 41.69 ± 19.27. We used Alloderm® as spacer graft of posterior lamella in 24 of 30 cases (80%), as wrapping material of exposed implants in 4 cases (13.3%) and as lower eyelid conjunctival spacer graft in anophthalmic contracted socket repair in 2 cases (6.7%). As spacer graft of posterior lamella we used Alloderm® in: dysthyroid lower eyelid retraction in 10 eyelids (33%), postblepharoplasty lower eyelid retraction in 5 eyelids (17%), eyelid neoplasia 5 eyelids (17%), gigants papilas of upper eyelid in 2 cases (6.7%), trichiasis 1 case (3.3%) and reconstruction of upper eyelid by palpebral agenesia 1 case (3.3%). There was clinical improvement when Alloderm® was used as spacer graft of lamella posterior in 79% (19/24 eyelids), no ophthalmic contracted socket showed improvement. Only one case 25% (1/4) of wrapping implant improved clinically. The complications found were often: retraction of Alloderm® in 6/30 cases (20%), symblepharon in 3/30 (20%) and extrusion in 2/30 (6.7%) . Follow up range from 3 months to 5 years (mean 10.6 months).
Conclusions: The Alloderm® is effective and safe as spacer graft of posterior lamella. It is an alternative for cases where other grafts can’t be used.
