ABSTRACT
A 47 year old male with normal ocular motility had a retinal detachment of the right eye that required a retinopexy with a 540 sponge, cryotherapy, vitrectomy and gas exchange in the vitreous cavity with C3F8 tamponade diluted at 15%.The patient had minimal inflammatory signs with a normal ocular motility without limitations or over action of extra ocular muscles. After 30 days of surgery when he was laughing out loud he felt something weird in his right eye. Next day he woke up with important blefaroedema, hypophthalmus, crepitation of the periorbitary tissue, vertical diplopia and chin elevation. A TAC was taken showing integrity of the orbital walls and diffusion of the gas within the orbital tissue.
The reabsorption of C3F8 has bee very slow. 8 months after the gas leak we have a gradual improvement of the periorbital and palpebral edema with a slight correction of the hypophthalmus and ocular elevation.
