Aberraciones de alto orden, sensibilidad al contraste y agudeza visual: estudio comparativo entre lentes intraoculares asféricos y convencionales

ABSTRACT
Purpose: To compare High-Order Aberrations (HOA), contrast sensitivity and visual acuity between aspheric and conventional intraocular lenses (IOL)
Methods: Controlled clinical essay. Patients with bilateral cataract and astigmatism ≤ 1D were included. Subjects with any other ocular disease were excluded. Phacoemulsification surgery was performed in a randomly chosen eye with implantation of an Alcon Acrysof Natural SN60AT IOL. Posteriorly the contra-lateral eye was submitted to the same procedure and an Alcon Acrysof-IQ SN60WF IOL was implanted. Visual acuity was measured under photopic conditions 80-100 cd/m². Contrast sensitivity was obtained with a sinusoidal Vistech CSV-1000 chart under photopic conditions 80-100 cd/m². Only high-order aberrations root mean square (RMS) was obtained with a WASCA (Carl Zeiss Meditec) aberrometer with 6 mm pupillary diameter and mesopic conditions 5-10cd/m².
Results: Thirty eyes (15 patients) were studied. Non-corrected visual acuity (NCVA) in group 1 (SN60AT) was 20/20 in 6 eyes (40%) and 20/15 in 9 eyes (60%), SD ± 0.152. In group 2 (SN60WF) the NCVA was in 3 eyes = 20/20 (20%) and in 12 eyes = 20/15 (80%), SD ± 0.124 p › 0.05. The mean RMS of HOA in group 1 was 0.68 µm (SD ± 0.167) and in group 2 = 0.42 µm (SD ± 0.15), p ‹ 0.05. The contrast sensitivity was higher in all spatial frequencies in group 2 with a p ‹ 0.05 in each of them.
Conclusions: The use of aspheric IOL results in diminished HOA and better contrast sensitivity compared to conventional IOL’s. No significant difference exists in regard to the visual acuity.
