ABSTRACT
Objective: To assess the results of transscleral fixation of posterior chamber intraocular lenses (PCIOL) in patients with inadequate capsular support.
Patients and methods: Prospective, descriptive trial of 24 eyes of 22 patients from February 2004 to February 2005 using transscleral fixation of an IOL. Patients cases consisted of :10 aphakic patients with past history of intracapsular cataract extraction, 2 patients with zonular dialysis following phacoemulsification surgery, 1 ocular trauma with luxated nucleus to vitreous, 1 patient with a luxated nucleus to vitreous post phacoemulsification surgery and 10 patients with inadequate capsular support due to surgical complications. Patients were excluded if they suffered from a low endothelial count, uveitis, glaucoma, proliferative retinopathy and had the use of only one functional eye. The follow-up was 6 months
Results: Average age was 68.18 years ( 55 to 89 years). Post-operative visual acuity (VA) was 20/40 or better in 18 patients (75%). Four patients did not improve VA due to an intraoperative complication (ciliary hemorrhage), post-operative retinal detachment, age-related macular degeneration and macular cystic edema. There was no statistically significative difference in intraocular pressure. All patients had an acrylic posterior chamber intraocular lens sutured.
Conclusion: PCIOL transscleral fixation results in an adequate surgical alternative for patients lacking sufficient capsular support
