ABSTRACT
Tuberculosis is still a worldwide health problem. Even in developed countries, its frequency has risen because of the AIDS epidemic. In Mexico, tuberculosis still represents a grave problem and, according to the reported statistics, an increase in the number of new cases, specially in the young adult population, has occurred. The prevalence rate is estimated in 51.7 cases per 100 000 people. Tuberculous otitis media exists in our countries, and is far more common that expected. Although great presentation variability makes the diagnosis a difficult one, the knowledge that this disease exists can be a very important factor in its early diagnosis and treatment. We present the case of a 40 year old male, who was sent to the Internal Medicine Department by Otorhinolaryngology for a pre-op (mastoidectomy) evaluation. The patient presented with chronic cough, intermittent nightly fever of 102º F, throbbing left ear pain, bilateral hypoacusia, right ear 25% and left ear 50%, Rinne (-), right ear subtotal tympanic perforation and was diagnosed by Otorhinolaryngology as drug resistant supurative bilateral otitis media. A middle ear biopsy was performed under direct visualization, with a middle ear lavage that was AFS positive; with a final diagnosis of tuberculous otitis media.REFERENCES
