ABSTRACT

Introduction: The tumors of the jugular glomus turn out to be very bleeding. For their boarding it is necessary to dissect the package vasculonerve of the neck, the lateral breast and the inferior petroso breast, that which makes that this to proceed is of difficult access. Their localization and the transoperatory bleeding are the situations dramatic to the one that faces the anesthesiologist. They require for their extirpation lingering surgical times, I don´t use of neuromuscular relaxants neither of nitrous oxide. Objectives: To specify the propofol use and fentanyl like anesthetics in the surgical extraction of these tumors. To determine if it is necessary the predeposit of blood autologous and the realization of hemodilution normovolemic. As well as, if some adverse reaction existed to the use of this drugs. Material and method: We carry out in the hospital Hermanos Ameijeiras in City of the Habana Cuba, 8 patients for the extirpation of jugular glomus. It is necessary the predeposit of blood autologous. We carry out hemodilution normovolemic with balanced electrolytic solutions, dextranos of high molecular weight and jellos. We reserve the packages of concentrated of red cells package for when indication exists for its administration. In all the patients we administer anesthesia general total endovenosa with Propofol 2 to 2.5 mg/kg in skittles and the maintenance we carry out it with continuous infusion of propofol to reason from 6 to 12 mg/kg/hora and opioids. Results: Three patients were feminine. The ages were 42 year-old stocking. The surgical time of 7.35 hours. All the patients had appropriate stability hemodynamic. Memories neither other complications neither adverse reactions didn´t exist with the use of this to proceed. Conclusions: This is a to proceed surgical of great magnitude that it implies high risk that requires a strict monitorization and the very established knowledge of particularities to minimize the complications that can be serious and it requires of a work team to carry out satisfactory.
