Frecuencia de miocardiopatía en neonatos de mujeres diabéticas

ABSTRACT
Objective. To determine neonatal morbimortality in infants of diabetic mother (IDM) and to determine the incidence and evolution of diabetic cardiomyopathy in the first six months of extrauterine life.
Material and methods. Twenty nine IDM were studied prospectively evaluating the cardiorespiratory status and the presence of congenital malformations. All were studied with serum levels of glucose, calcium, magnesium and bilirrubin, furthermore central venous hematocrit in first 48 h of life, a soon as, electrocardiogram and Doppler echocardiography at birth, three and six months of life. Three criterion were examined: ventricular septal thickness, left ventricular posterior wale size and left ventricule out flow.
Results. Four patients had respiratory failure: two with respiratory distress syndrome and two with tachypnea of the newborn. One patient had cardiac failure with supraventricular tachycardia. Eighteen (0.62) patients had hypoglicemia, 12 (0.41) hypomagnesemia, eight (0.28) hyperbilirrubinemia, five (0.17) hypocalcemia and three (0.10) polycitemia. Fifteen (0.52) presented total cardiomyopathy and two (0.06) apical cardiomyopathy with no obstruction. All improving of this condition in the six months of life. There was not mortality.
Conclusion. The metabolic disturbance and the diabetic cardiomyopathy, are frequent in the morbidity of IDM. We suggest a complete evaluation in this group of newborn.
