Lumbosacral agenesis: Treatment and proposal of a new classification

ABSTRACT
Introduction: The lumbosacral agenesis is an uncommon congenital anomaly. Material and methods: We were revised from 1990 to 2003 all the patients with a diagnosis of sacral agenesis and we evaluated the type of agenesis, family background, spinal and limbs malformations that are related with this pathology. Patients were evaluated with a new classification: Type I: partial agenesis, Type II: total agenesis. Patient with spinal-pelvic instability were treated with a new technique of arthrodesis. Results: 21 patients were studied: 10 men and 11 women. Relating lumbosacral agenesis with a diabetic mother, 13 (62%) patients had healthy mothers and 8 (38%) patients had a diabetic mother. The following of the patients was an average of 3.1 years (range 0.1 to 17), with a total average following of 5.2 year-old average (range 1.0 to 12.8). Nine patients (43%) had a community gait without use of ortesis, another 9 patients (43%) had a community gait with use of ortesis and 3 patients (14%) were mobilized with a wheelchair. Sixteen patients (76%) had thoraco-pelvic stability and 5 patients had thoraco-pelvic instability, reason for which they were underwent to a thoraco-pelvic arthrodesis with good results. Conclusion: The lumbosacral agenesis is a pathology difficult to try and when accompanies with myelodysplasia it is much more difficult to treat the spinal-pelvic instability. For the pelvic instability thorn the described technique provides a good alternative.REFERENCES
