Treatment of the hyperglicemic hyperosmolar non ketotic state with low-dose insuline infusion

ABSTRACT
Objective: To determine the effectiveness of low-dose insulin infusion for the treatment of nonketotic hyperosmolar state (NKHS).
Design: Case series report.
Patients and methods: Twenty patients from the emergency room department with hyperosmolar state were allocated in two groups: 1) short acting insulin infusion at 0.1 IU/kg/h, 2) A scheme of short acting insulin according to capillary glucose as follow: 180 = 4 IU, 240 = 8 IU, more 400 = 10 IU. We carried out determinations of serum glucose, serum osmolarity, hemodynamic parameters, neurological state at admission and at 2 and 8 h; Student t and square chi were used as statistics analysis.
Results: At 8 h group 1 had a serum glucose of 293 mg/dL, capillary glucose 271 mg/dL, serum sodium 42 mmol/L, serum osmolarity 301 mmol/kg. In group 2 serum and capillary glucose weere 506 mg/dL and 442 mg/dL respectively, serum sodium 153 mmol/L and serum osmolarity 345 mmol/kg (p ‹ 0.01).
Conclusion: The use of low-dose insulin infusion was more effective than the traditional insulin treatment in the recovery of patients with non ketotic hyperosmolar state.
