Attention deficit disorder and comorbidity with addictions in a cohort of children, adolescents and adults

ABSTRACT
Introduction: Several retrospective studies suggested an association between substance abuse and attention deficit disorder with hyperactivity (ADD/H). In Mexico there are no publications that demonstrate the co-morbidity with addictions, affective disorders and anxiety. Our objective was to evaluate the presence of ADD/H in a population of an Attention Disorder Clinic and correlate the disorder and the co-morbidity with substance abuse, affective disorders and anxiety.
Material and methods: We studied 112 patients prospectively with the presumptive diagnosis of ADD/H who attended a specialized Clinic of ADD/H in the period of two years. The diagnosis was confirmed in 78 patients. Thirty two were children, 12 adolescents and 34 adults. We correlated the presence of ADD and the co-morbidities with a history of addictive behavior, affective disorders and anxiety. We also demonstrated the presence of addictive behavior and type of ADD present in the patients.
Results: We found that 4 of 12 adolescents (30%) had a history of use or abuse of drugs. A high percentage of adults diagnosed as having ADD had a history of drug abuse (88%). The proportion of men and women was 3:1. There was a high proportion of co-morbidity with anxiety and affective disorders. The majority had an addiction to multiple substances (76%) a high percentage had ADD of impulsive or mixed type.
Conclusions: There is a high correlation between ADD and substance abuse in the adult population. This correlation is more evident with males; impulsivity, affective disorders and anxiety. A large proportion of patients used multiple substances.
