Quality care in an intensive care unit

ABSTRACT
Objective. To establish a comparison of the quality care between an open ICU versus a closed ICU.
Design. Retrospective, comparative study.
Setting. A second level ICU of a general hospital of Mazatlán, México.
Patients. We reviewed the records of all consecutive ICU admissions during five years. A comparison of the quality care (utilizing the Donabedian’s model) of our ICU, from January 1991 to December 1992 (open ICU, minimal resources) and January 1993 to December 1995 (closed ICU, major resources) was established.
Interventions. None.
Measurements and main results. Between the first and second steps, an increase of total yearly admissions (301 vs 358; p ± 0.001), occupancy rate (23% vs 75%; p ± 0.0001), renewal index (26.2 vs 56.4) and ICU stay (3.3 vs 4.5 days; p ± 0.01) was observed. The mortality rate decreased from 42% to 21% (p ± 0.00001) and the mortality on the 1st ICU day descended from 23.3% to 7.6% (p ± 0.00001).
Conclusion. An open ICU with better resources may increase the quality of care in our city.
