Abdominal sepsis and tamponade. Case report

ABSTRACT
Objective. To report a case of abdominal sepsis complicated with cardiac tamponade.
Design. Case report.
Setting. An ICU of a second level hospital of Mexico City.
Patient. A 15-years-old male was admitted to the ICU with diagnosis of septic shock twelve hours after a drainage of pelvic abscess secondary to acute appendicitis.
Interventions. Vital support, hemodynamic monitoring.
Measurements and main results. Leukocytosis, widening of mediastinum, pericardial effusion (500 mL) cardiac arrhythmias and hemodynamic unstability was observed. A pericardiotomy was performed and the patient was discharged to a surgical room at 21 ICU day.
Conclusion. Cardiac tamponade is a rare complication of abdominal sepsis. Unexplained hemodynamic unstability maybe a clue for diagnosis.
