Prevalence of depression in patients with diabetic foot

ABSTRACT
Background: Depression is the most common psychiatric consult in the medical practice, it affects almost 10% of the overall population at some time during their life time. It is reported to be present in 18 to 35% of the patients from general hospitals. In some chronic diseases, it’s been proved that depression can deteriorate their prognosis.
Objective: To describe the prevalence of depression among patients with diabetic foot and to analyze the relationship between the functional capacities of the patient and the clinical, social and demographic variables with the presence of depression.
Methods: This is a prospective, transversal and analytic study, including patients with diabetic foot who were treated at our hospital from March to August 2004. We used the Zung Scale for depression, the Katz questionnaire for the functional status, and we also collected clinical and demographic data. The statistical analysis included chi square test, t Student and logistic regression.
Results: We included 57 patients, 37 (64.9%) male, median age of 60.4 years (45-77). Depression was diagnosed in 26 patients (45.6%). The variables that showed a significant statistical association with the presence of depression were: functional status (p ‹ 0.005), Wagner scale (p ‹ 0.001) and the duration of the illness (p ‹ 0.005) IC 95% (2.06-10.82)
Conclusions: The prevalence of depression in patients with diabetic foot is high. Deterioration in the functional status, an advanced stage in the Wagner scale and a long duration of the illness were the factors significantly associated with the presence of depression. A psychiatric evaluation in the group of patients who present these characteristics is important, with the purpose to lessen the increase of morbidity and mortality that is caused by the mood alterations.
