Anatomic and functional aspects upon Broca's area in functional neurosurgery

ABSTRACT
In the current review, some aspects of the anatomy of the Brocas´s area like clinical expression of the Broca´s aphasia are discussed, including the alteration in the understanding of the oral language. Also, functional findings with the use of recent techniques are exposed, like the Functional Magnetic Resonance. One concludes that: 1) anatomically there is no a consensus with respect to the cerebral cortical limits of Broca´s area, 2) the Broca´s area traditionally has been related only to the expression of the language; nevertheless, it is also involved in language comprehension, although still is not known the exact way of its participation, mainly in spanish speakers, 3) it is a region that participates in multiple functions, like the reading and the writing, although it is not know the exact way of its contribution and 4) the present techniques of neuroimaging have allowed to verify several aspects already mentioned by some authors. Finally, there is scientific evidence that an injury circumscribed to the Broca´s area does not produce a severe and permanent language disturbance, which allows neurosurgical procedures denominated multiple subpial transections in this area, because it gives the possibility of cerebral reorganization.
