Analysis of heart rate variability in a group of patients with renal failure treated with chronic hemodialysis

ABSTRACT
Background: Hemodialysis (HD) hypotension is a very complex phenomenon; it depends on multiple factors, all of them precipitated by ultrafi ltration process. Previous hemodyalisis issues have demonstrated poor sympathetic response in patients with chronic renal failure (CRF). However it is unexplained the role of the heart rate variability (HRV) as a marker of autonomic system balance.
Objective: To show the heart rate variability analysis pre and post hemodialysis, and correlation with urea kynetics assessed by urea reduction rate (URR) and KT/V, and to explain the effects of the sympathetic and parasympathetic components of HRV in crF patients with HD treatment.
Patients and methods: It was performed a transversal, decriptive study in a cohort of 11 patients in chronic hemodialysis, patients were their own control pre and post hemodialysis. Biochemical studies were done pre and post hemodialysis to obtain results of urea kinetics.
Results: The most frequent cause of CRF was diabetic nephropathy (81.8%), the mean age was 59 ± 9.4 years, the urea reduction rate 63 ± 6.08%, and KT/V 1.19 ± 0.16. All of the heart rate variability components were reduced, with not signifi cative difference pre and post hemodyalisis. We found increased parasympathetic tone measurement by LF/HF index ›1.
Conclusions: Hemodialysis hypotension pathophysiology is a multifactorial aspect. The autonomic system is not depending of uremia, patients with chronic renal failure in hemodialysis treatment have disautonomic response characterized by reduced sympathetic tone and increased parasympathetic.
