Osteochondrytis of the astragalus. Criteria for the surgical treatment

ABSTRACT
Objective. To recover complete physical activity by performing surgical treatment of talus osteochondritis. Material and methods. Antero-medial or antero-lateral arthrotomy of the ankle joint was carried out in 14 patients aged 18 to 39 years. Osteotomy of the medial malleolus was made when lesion was found to posteriorly. All patients used to practice sport and in 80% there was history of trauma including one or more ankle spains. X-rays demonstrated an antero-lateral subchondral fracture in five cases and postero-medial in nine. CT demonstrated osteochondritis in the same pattern of frequency. Indication for surgical treatment was given by pain rather than radiological findings. In stage I, bone drills were the unique treatment, while in stages II, III and IV, removal of a sequester lesser than 1.5 cm and curetage of nechrotic bone were performed. One case who had a loose viable major bone fragment, fixation was done by a pin and in three in whom a major fragment was not viable, this was removed and carbon cilynders was applied once cureted nechrotic bone. Results. Were good ranging between 87 and 100 points according to the AOFAS scale. Conclusion. Surgical treatment is expected to give good results, whennever indicated according to the severity of symptoms in osteochondritis of talus.REFERENCES
