Tibial plateau fractures. The importance of the classification for the diagnosis, the treatment and the prognosis

ABSTRACT
Tibial plateau fractures include the articular cartilage, epiphysis and metaphysis. The treatment is controversial, today it tends to be surgical and has the goal to get stability, alineation and avoid degenerative changes. We analyze the tibial plateau fractures managed in the ABC Medical Center from March 1995 to November 1998, including those with articular component and excluding those without articular component, isolated condral fractures, and patients without following possibilities. Thirty-nine patients was studied, 27 males and 12 females (middle age 44.1 years). The results were analyzed in base of Schatzker classification. Conservative treatment has 67% good results for type I fractures. Surgical treatment was made in 92.65% of patients. The treatment with small incisions and screws by fluoroscopy is adequate for types IV and I and most not be used in fractures with depression. We recommend arthroscopic treatment in fractures without displacement or in simple fractures with light depression. Massive elevation with bone graft internal fixation is the choice treatment for Schatzker II, III, IVB, V and VI. Complication was deep infection 2 cases, no consolidation 3 cases, and depression in 6. Classification of tibial plateau fractures give us a diagnostic, therapeutic and prognostic guide in a controversial fracture.
