Post-traumatic coxarthrosis in the acetabulum complex fractures

ABSTRACT
Post-traumatic coxarthrosis is frequent in complex acetabulum fractures. We plan to investigate the effect on the results of the degree of reduction and the time elapsed between the fracture and the moment of surgery. We developed a study of recorded cases from January 1996 to December 1997, which included 20 complex acetabulum fractures (Judet and Letournel) treated within three weeks after lesion. We classified the quality of reduction like anatomical, satisfactory and displaced (Matta). The clinical evolution and radiographic evolution were assessed using the Merle D’Aubigné and Postel criteria modified for Matta. The patients’ average age was 33 years. The elapsed time between the fracture and the surgery was 11 days. Both columns fractures prevailed. The averaged follow up was 17 months. We achieved anatomical reduction in 8 cases, satisfactory in 10 and displaced in 2 cases. Our results to clinical evaluation were: one excellent, 15 good, 2 fair and 2 poor. And in the radiographic evaluation we had: 2 excellent, 8 good, 9 fair and one poor. We found that a better reduction, means better clinical and radiographic results (p ‹ 0.001). The age and elapsed time from lesion to surgery didn’t influence on the results (p › 0.05). We conclude that the type of reduction is one of the most important factors in the patient prognosis.
