Pyogenic arthritis in pediatrics

ABSTRACT
Patient with septic arthritis involves a medical and surgical emergency. It is very important to promptly establish the diagnosis of septic arthritis and give adequate treatment to avoid sequelae. In this paper we report our experience in 160 patients and risk factors for the development of complications are evaluated. The knee was the most common joint involved. The most common etiologic agents isolated were
Staphylococcus aureus (all ages),
H. influenzae (under two years) and enterobacteria (newborns). Diagnosis of septic arthritis was established with arthrocentesis; the erythrosedimentation rate and reactive C-protein were helpful adjuvants in the diagnosis and follow-up of patients with septic arthritis. Initial treatment was always with IV antibiotics. Every patients was evaluated to decide whether or not to perform a surgical arthrotomy. In this study 92% of the infants had complications, and all of them had a clinical course of more than 7 days of duration. This emphasizes the importance of an early diagnosis of the disease.
