ABSTRACT
Bone infections in orthopedic surgery represent a serious problem, in consequence preventive parenteral antibiotic have been used, having the inconvenience that they arrive in little concentration to the surgical wound. Efforts to revert this fact are: the use of surgical cements containing this medications, continuous infusion systems and lately the collagen and gentamicin sponges; methods that take the antibiotic to the place of the wound in high concentrations. From June 1996 to September 1997, 80 patient with fluctuating ages from 6 to the 92 years, with a 68 year-old average were studied; they were 41 female patients and 39 male patients. With an averaged follow up of one year. They were performed 80 diverse orthopedic procedures where the sponge with gentamicin was used to help systemic antibiotics. We found that, until the moment of the present study, we do not have postoperative infections and that the urine levels of gentamicin in 36 patients did not reach toxic levels for this medication allowing its use of this type of sponges with gentamicin in patient with renal problems.
