Nucleotome percutaneous debridement for pyogenic spondylo-discitis

ABSTRACT
Twenty two patients with spondylo-discitis piogenic of lumbar column were studied, being 15 men and 7 women, with a 54 year-old average. In two thirds of them it was found the infection focus, prevaling:
Salmonella, Brucella and Gram-positive coccus. The affected level was L3-L4 and clinically all had a sciatica systematized to some dermatome and the radiological studies showed a destructive bone process in platforms with sclerosis reaction, bone spicules, neurological compression for the abscess and segmental instability. Traditionally there are two treatment types: the conservative, with antibiotics and rest in bed until for nine months; and the surgical one with a high risk of morbi-mortality. We propose another viable alternative of treatment, with the use of the Nucleotom for the percutaneous drainage of the abscess, to be non invasive and to provide a sample for the bacteriological and pathological study. With the percutaneous drainage of the abscess, the neurological structures and the sciatica are decompressed it remitted in one to five days, with progressive neurological recovery of one to four weeks, in 95% of the cases. When draining the abscess it was observed a prompt repair of the bony defect and an spontaneous coalition of interbody vertebrae et 4-5 months in the 22 cases, stabilizing the column.
