Arthroscopic treatment for gonarthrosis and tibial osteotomy with external fixators. Personal experience in 40 knees

ABSTRACT
Arthrosis of the knee was treated in 32 cases older than 50 years, with 40 knees by arthroscopic debridement. A proximal tibial osteotomy which was fixed by an external fixator was performed in the same operative time in 10 cases who had angular deformity. Debridement consisted on reaming, drilling and synovectomy. Intra-articular bupivacaine and dexametasone was employed at the end of operation in order to decrease the risk of postoperative intra-articular adhesion and to avoiding excessive pain. Most common degrees of arthrosis were IV to VI. Postoperative results were excellent in 20 cases, good in 10, fair in 2 and poor in 2. Complete recovery was achieved after an average of 7 months in most patients.
