Arthrofibrosis of the knee: epidemiology and arthroscopic surgical treatment

ABSTRACT
Arthrofibrosis of the knee was found in 24 patients aged in average 36.4 ayears (17 to 62) as a result either of cruciate ligament plasty, long-lasting rest, infection, inspecific synovitis, distal femoral or proximal tibial fractures and other causes. Patients were treated by gentle manipulation under anesthesia without effective result. Then arthroscopy for release of intra-articular adhesions was carried out through 2 standard infero-medial and infero-lateral portals and in most cases a third supra-lateral approach for release of the supra-patelar recess. An exercise program and early weight-bearing was encouraged in mild cases while in sereve the continous pasive motion device and cryotherapy was applied for the first 10 days followed by active excercises of the knee. Results demonstrated an improvement from 4 to 30 degrees after artroscopic release and excercise treatment in severe cases of arthrofibrosis.
