ABSTRACT

Thoracoscopic surgery is indicated in patients that require an anterior approach to the thoracic spine and who have a high risk for open thoracotomy. We report two patients with post traumatic vertebral body burst fractures, with spinal cord compression. Both cases underwent thoracoscopy and corporectomies to achieve medullar canal decompression; an autologous bone implant was obtained and placed inside a titanium mesh to achieve intersomatic arthrodesis and internal fixation with a Z plate. This approach is less traumatic than an open thoracotomy, allowing the surgeon to make minimal incisions and to have a better view by means of image magnification. Although our long-term results were not those expected, they were encouraging, since they showed corporectomy, arthrodesis and anterior thoracic instrumentation by thoracoscopy are accessible, in selected patients, for the treatment of thoraco-lumbar fractures. Use of thoracoscopy could lead to achieving the same benefits an open thoracotomy offers, and we consider it a useful option in trained hands.
